Thank you for your interest in stocking my patterns in your store.  Before I can approve you as a wholesale merchant I need to know a little bit about yourself and your business.  Please email me with the following info and I will get back to you as soon as I can.

• What is your name and email address?
• Do you have a physical store?  If so, what is it called and where is it located?
• Do you have an online shop/website?  If so, what is your website address?
• Where do you intend to retail my patterns? (eg physical store, online store, markets, eBay etc) – Please list all intended locations.
